Disease management is an emerging approach to patient management, customer satisfaction, and cost containment. It is estimated that over 80 million people in the U.S. have diabetes or are on the verge of developing the disease. The total cost of diabetes in the United States in 2007 was $218 billion, factoring in the additional costs of undiagnosed diabetes, pre-diabetes, and gestational diabetes. Implementing a disease management program poses
significant challenges to healthcare organizations. A robust
diabetes management with patient self management is key to
increase compliance and drive down cost. The current toolsets
that are available today require patients to manually enter
their blood sugar numbers. These systems are not user friendly
and are so complicated that patients cannot use the tools. These
systems have a closed loop approach and will not share information between devices and other manufacturers, which causes the patients
more inconvenience, resulting in the lack of adoption of these tools. The end result is that patients are not adequately monitoring or managing their diabetes thereby reducing their overall health and increasing healthcare costs.
S5Health has set itself apart from the current offering of products by diabetes device manufacturers and other diabetes portals in the following ways:
Support for all devicesS5Health has a unique business model compared to the diabetes management offering of the mega-players
their business models are designed around purchasing their products and devices and then implementing their solution from beginning to end. In actuality, healthcare payors, providers and patients use a mix of devices from a variety of manufacturers. The diabetes management portal is manufacturer neutral and supports all pumps and meters by leveraging print files from all devices. Patients upload these reports into S5Health'S central system where the information is consolidated and normalized into the portal's database. The diabetes management portal allows patients, providers and users to view the data that is important to them and allows them to view the data in the manner most useful to them.
Ease of use and intuitive dashboards
The dashboards and reports from the mega-players are so complicated they are neither readable nor useful to patients or
payors. These reports are also static and not interactive so patients and
payors cannot drill deeper into the information to find root
causes of problems. Our diabetes management portal allows
patients and payors to easily see trends and drill into times of day so problem areas can easily be identified and the root cause of problems addressed, thereby improving the health of diabetes patients.
Community portal
Healthcare is local and so is the community of people in which diabetic patients want to interact. The diabetes management portal allows diabetic patients and caregivers to interact online and find offline events where they can meet in person and learn and find comfort from other local families dealing with the challenges of diabetes.
Measures the effectiveness of treatment
Currently, the measurement of how well patients manage their diabetes is to test their HbA1c levels every 3 months. The industry realizes this test is not perfect and only shows the averages of blood sugars over a period of time. For example, the HbA1c of a patient with 30 blood sugar levels of 300 (hyperglycemic or high blood sugar) and 30 blood sugar levels of 50 (hypoglycemic or low blood sugar) would show the same HbA1c level as a patient with 60 blood sugar levels of a near perfect 125, yet the patient with high and low blood sugars is poorly managing their diabetes. The diabetes management portal stores all blood sugar readings and changes to dosage and insulin levels so patients and providers can measure the results of the changes made to these levels. By measuring the results of these changes in near real time, treatment and dosage levels can be best matched to improve blood sugar levels of patients thereby improving their health and reducing healthcare costs.
Brandable to the Customer Environment
To drive adoption throughout a company and to integrate into existing portals, our diabetes management portal will be brandable to a company. From our CEO’s experience, branding is very important to customers in a SaaS offering and the adoption of the solutions. Users want a consistent interface and don’t want to be “bounced” between portals when using online tools.
